  • Reconstitution example (common 10 mg vial): Add 3 mL bacteriostatic water → ~3.33 mg/mL. – 1 unit (0.01 mL) on U-100 insulin syringe ≈ 33.3 mcg. – 200 mcg ≈ 6 units (0.06 mL) – 300 mcg ≈ 9 units (0.09 mL) – 400 mcg ≈ 12 units (0.12 mL) – 500 mcg ≈ 15 units (0.15 mL)

  • Alternative (5 mg vial example): Add 1–2 mL bacteriostatic water for 2.5–5 mg/mL concentration; adjust units accordingly (e.g., 10–20 units for 100–200 mcg in some pre-mixed strengths).

  • Duration: Often 7–21 days or until desired outcome; some extend 4–8 weeks with titration.
